Acute Care Visits – Complete, same-day assessments and treatment for illness or injury. POC Test included in visit if appropriate.
Wellness & Weight Loss Consultations – Personalized guidance to help you reach and maintain your health goals.
Care Coordination – Collaboration with your PCP or specialists for services such as wound care, IV administration, and other nursing or provider-ordered treatments.
Lab Orders & Prescriptions – Conveniently managed by your provider, with labs and medications billed directly to you by the lab or pharmacy for full cost transparency.
An initial visit will be completed to ensure your needs and provider orders can be safely managed in the home setting, particularly for subacute or chronic conditions.
